So, the Permit is completed and ready for the Wellsite Permit Authority (WPA) to review, authorise and issue. A key aspect to this task is for the WPA to clearly identify the Required Attachments noted on the Permit and verify that all the attachments, including risk assessments are included with the Permit documentation.
Required Attachments are those documents, other than the Permit Form that are required to properly document the work activities, Work Precautions and Ongoing Control requirements for the Scope of Work being permitted.
It is the WPA responsibility to confirm that any Work Precaution that identifies the need to attach a document has been selected in the Work Precautions and Required Attachments sections.
Additionally, the WPA must validate that attached documentation fully represents the documented Scope of Work and identified Work Categories.
Where generic Job Safety Analysis (JSA) or Standard Operating Procedures (SOPs) are provided, the WPA must review the document to verify that the JSA or SOP reflect any changes to conditions or the environment. This can be as simple as marking up these documents with required changes. All changes must be communicated to the Wellsite Permit Holder (WPH) and Work Party.
Attached documents should remain with the Permit Form and are surrendered to the WPA at Closeout.
